Do Systemic Inflammatory Markers Reflect Orbital Invasion in Periocular Basal Cell Carcinoma?

Purpose
To compare systemic inflammatory markers between patients with isolated periocular basal cell carcinoma (BCC) and those with orbital involvement, and to investigate their association with clinical parameters.
Methods
This retrospective study included 51 patients with histopathologically confirmed BCC diagnosed between 2020 and 2026: 39 with isolated periocular involvement (Group 1) and 12 with orbital involvement (Group 2). Additionally, 39 age- and sex-matched healthy individuals were included as controls (Group 3). Demographic data, pretreatment complete blood count parameters, and systemic inflammatory indices — including neutrophil-to-lymphocyte ratio (NLR), monocyte-to-lymphocyte ratio (MLR), platelet-to-lymphocyte ratio (PLR), systemic immune-inflammation index (SII), systemic inflammation response index (SIRI), aggregate index of systemic inflammation (AISI), and the HALP score — were compared among groups. Associations between inflammatory markers and clinical features were also evaluated.
Results
Demographic data were comparable among groups. Both BCC groups had significantly higher NLR and SIRI values than controls (p=0.002 and p=0.002, respectively). Furthermore, patients with orbital involvement had significantly elevated MLR (p=0.036) and SII (p=0.032) than controls. Although not reaching statistical significance, Group 2 consistently showed higher mean values across PLR, NLR, MLR, SII, SIRI, and AISI compared to Group 1. Inflammatory markers did not differ significantly among BCC subtypes (p>0.05). Disease duration showed a significant positive correlation with PLR (r=0.395, p=0.017) and SII (r=0.455, p=0.005), and a negative correlation with the HALP score (r=−0.409, p=0.013). Tumor diameter and volume were not significantly correlated with inflammatory markers.
Conclusion
Systemic inflammatory markers are elevated in BCC, with orbital involvement tending to reflect a more pronounced inflammatory profile than isolated periocular disease. Simple, cost-effective markers such as NLR, SIRI, MLR, SII and HALP score may aid in recognizing orbital extension.
