Upper Eyelid Smooth Muscle Tumor of Uncertain Malignant Potential (STUMP): A Rare Cause of Mechanical Ptosis.

Purpose
To report an exceedingly rare case of upper eyelid STUMP presenting as mechanical ptosis and to highlight the diagnostic challenges of mesenchymal tumors and the role of histopathology.
Methods
Observational case report with clinical, radiologic, and histopathologic correlation.
Results
A 47-year-old female presented with a progressively enlarging right upper eyelid mass associated with mechanical ptosis and superior visual field defect. Clinical examination revealed a firm, well-defined, mobile lesion without evidence of orbital involvement. MRI showed a well-circumscribed soft tissue mass without bony erosion or deep orbital extension; however, a neoplastic process could not be excluded. Complete surgical excision was performed. Histopathologic analysis demonstrated spindle cell proliferation with mild to moderate cytologic atypia, low mitotic activity, and absence of tumor necrosis, consistent with STUMP. Systemic evaluation, including gynecological assessment, was unremarkable. The postoperative course was uneventful, with complete resolution of ptosis and no evidence of recurrence at short-term follow-up.
Conclusion
STUMP of the eyelid is exceedingly rare, with only isolated reports of extrauterine involvement in the periocular region. To our knowledge, STUMP arising in the upper eyelid has not been previously reported. These lesions may clinically mimic benign eyelid masses, contributing to diagnostic uncertainty, while imaging findings are often nonspecific and unable to reliably distinguish benign from potentially aggressive mesenchymal tumors. As demonstrated in this case, definitive diagnosis relies on histopathologic evaluation. The origin is uncertain but likely from periocular smooth muscle such as Müller muscle or vascular smooth muscle.The absence of gynecological pathology further supports a primary extrauterine origin. Given the uncertain biological behavior of STUMP, complete surgical excision is essential, and long-term surveillance is recommended due to the potential risk of recurrence or progression.
