Management of Lower Eyelid Basal Cell Carcinoma with Hughes Flap Reconstruction

Purpose
To present the surgical management and reconstructive approach in a patient with lower eyelid basal cell carcinoma, highlighting functional and aesthetic outcomes following combined flap techniques.
Methods
A 69-year-old male with a history of systemic hypertension and Transient Ischemic Attack presented with a persistent right lower eyelid lesion initially diagnosed as chalazion. Ophthalmologic examination revealed a vascularized nodular lesion with hemorrhagic crust, suspicious for malignancy. A complete excisional biopsy with 4-mm safety margins was performed, followed by immediate reconstruction using combined Hughes Flap and Tripier Flap techniques to restore posterior and anterior lamellae.
Results
Histopathology confirmed a solid subtype of Basal Cell Carcinoma measuring 1.4 × 0.5 cm with tumor-free margins. The procedure was completed without complications. Postoperative management included oral antibiotics and topical corticosteroids. Follow-up demonstrated no evidence of recurrence, with satisfactory eyelid function and cosmetic outcome.
Conclusion
Basal cell carcinoma is the most common periocular malignancy, characterized by local invasiveness and potential functional impairment. Complete surgical excision with clear margins remains the gold standard. Reconstruction of full-thickness eyelid defects requires precise technique selection; combined flap approaches, such as Hughes and Tripier flaps, provide reliable structural and aesthetic restoration. Early diagnosis and multidisciplinary management are essential to optimize outcomes and preserve ocular function.
