Long-term outcomes after external dacryocystorhinostomy according to Toti between 2017 and 2026

Purpose

The aim of this retrospective study was to evaluate the effectiveness of external dacryocystorhinostomy (DCR) according to Toti. Both early anatomical success and long-term functional outcomes were analyzed. In addition, long-term stability, possible recurrences, and postoperative complaints were assessed.


Methods

This study included all patients who underwent external DCR according to Toti over an eight-year period at our eye department. All procedures were performed by a single surgeon; a total of 94 eyes in 91 patients were operated on.Functional outcomes were assessed in a subsequent cross-sectional evaluation based on subjective patient reports, including the degree of epiphora (according to the Munk scale), recurrent inflammations, postoperative complaints, and overall satisfaction. In addition, patients were asked about the visibility and possible cosmetic relevance of the postoperative scar.


Results

The results were based on the assessment of epiphora using the Munk scale after an average follow-up period of 40–45 months. Depending on the localization of the preoperative stenosis, postsaccal obstructions showed the best outcomes, with a success rate of 95.7%.In presaccal and saccal stenoses, the functional success rate was 77.4%. Among the five patients who required reoperation, the success rate after the second procedure was 87.5%. Postoperative scarring was unproblematic: approximately 4.3% of patients reported a visible scar, which was not perceived as cosmetically disturbing.


Conclusion

The External DCR according to Toti proved to be a reliable and long-term effective procedure for the treatment of lacrimal outflow obstruction in our patient cohort. Particularly high success rates were achieved in postsaccal stenoses, while presaccal and saccal obstructions also benefited, albeit with lower functional success rates. Reoperated cases likewise showed predominantly positive outcomes. Postoperative scarring was rarely visible and did not represent a relevant cosmetic concern. The analysis confirms the high effectiveness and good tolerability of the Toti procedure.
