COMPUTED TOMOGRAPHY EVALUATION OF UNILATERAL NASOLACRIMAL DUCT OBSTRUCTION:A COMPARATIVE STUDY WITH THE CONTRALATERAL SIDE AND HEALTHY CONTROLS

Purpose

This study aimed to evaluate the morphological characteristics of the paranasal sinuses, nasal structures, and nasolacrimal duct using computed tomography (CT) in patients with unilateral primary acquired nasolacrimal duct obstruction (PANDO). Findings from the obstructed side were compared with the contralateral patent side and healthy controls.


Methods

The study included 22 patients with unilateral PANDO and 25 healthy individuals without epiphora. In patients, the obstructed and contralateral sides were assessed separately and compared with the right side of controls. Measurements included lacrimal gland anteroposterior and transverse diameters, inferior turbinate thickness and angle, and the anteroposterior/transverse diameters, cross-sectional area, and length of the bony nasolacrimal canal. Septal deviation and angle, agger nasi cells, concha bullosa, osteomeatal complex opacification, and maxillary, ethmoidal, and frontal sinusitis were also evaluated.


Results

The patient group (57.9 ± 16.06 years; 19F, 3M) and control group (59.8 ± 15.35 years; 21F, 4M) were similar in age and sex distribution (p > 0.05). The anteroposterior diameter of the nasolacrimal canal was significantly smaller on the obstructed side compared with the contralateral side and controls (p = 0.039). The narrowest cross-sectional area was also significantly reduced on the obstructed side (p = 0.027). Osteomeatal complex opacification and ethmoid sinusitis were significantly more frequent in patients (p = 0.026 and p = 0.033). Other parameters showed no significant differences (p > 0.05).


Conclusion

Reduced nasolacrimal canal diameter and area may contribute to PANDO development, while associated sinonasal inflammation may also play a role in disease pathogenesis.
