Pruning the roots: Role of preoperative ophthalmic artery embolization in Managing a Rare Case of Hypervascular Orbital Fibrous Tumor with Ophthalmomyiasis

Purpose

Solitary fibrous tumors (SFTs) are rare mesenchymal tumors of the orbit that are often highly vascularized. Their tendency to grow extensively can lead to complications such as ulceration, hemorrhage, infection, and, in rare instances, parasitic infestation. Surgical resection of large, hypervascular SFTs—particularly those extending to the orbital apex—poses significant challenges due to the increased risk of intraoperative and postoperative bleeding. This report presents a case of a large, hypervascular orbital SFT complicated by ophthalmomyiasis, managed with preoperative angioembolization to reduce surgical risks

Results

The patient underwent preoperative embolization via the ophthalmic artery using 100–300 µm and 300–500 µm embolic microspheres, delivered while a balloon catheter was inflated. The procedure was performed 48 hours before surgical excision. Angioembolization effectively reduced the tumor’s vascular supply, resulting in minimal intraoperative bleeding, shorter surgical duration, and an overall easier excision. No intraoperative or postoperative complications were observed.


Conclusion

This case demonstrates the utility of preoperative angioembolization in the surgical management of large, hypervascular orbital SFTs. By significantly decreasing intraoperative bleeding and facilitating tumor removal, this approach may enhance surgical safety and outcomes in similar challenging orbital cases.
