Real-world effectiveness and adverse events of Teprotumumab in Japanese patients with thyroid eye disease

Purpose
To evaluate the real-world effectiveness and safety of Teprotumumab in Japanese patients with thyroid eye disease (TED), focusing on clinical outcomes, quality of life, and adverse events.
Methods
A retrospective study was conducted in 7 Japanese patients with moderate-to-severe active TED treated with Teprotumumab. Active TED was defined as a Clinical Activity Score (CAS) of 3 or greater, or a CAS below 3 with MRI findings suggestive of active orbital inflammation. CAS, Hertel exophthalmometer measurements, ocular findings, and Graves’ Ophthalmopathy Quality of Life (GO-QOL) scores were assessed before and after treatment. Adverse events during and after treatment were evaluated.
Results
The study included 1 man and 6 women with a mean age of 56.3 years and mean disease duration of 15.6 months. Mean CAS improved from 1.9 to 0.6, and the CAS response rate was 100%. Mean Hertel exophthalmometer measurements improved from 18.4 mm to 17.1 mm, with a mean reduction of 1.79 mm; the proptosis response rate was 66.7%. Eyelid swelling, conjunctival injection, and pain improved markedly, whereas diplopia remained unchanged. Total GO-QOL score improved from 74.3 to 79.6, mainly due to psychosocial improvement. Adverse events included hearing impairment in 3 patients, diarrhea in 3, alopecia in 2, hyperglycemia in 1, and menstrual irregularities in 1. Hearing impairment in 1 patient, alopecia in 2 patients, and menstrual irregularities in 1 patient continued after treatment. Only hearing impairment needs therapeutic intervention.
Conclusion
Teprotumumab improved inflammatory signs, symptoms, and psychosocial QOL despite modest reduction in proptosis in Japanese patients with TED. Compared with the OPTIC-J trial, the proptosis response rate was lower whereas the CAS response rate was higher. These differences may reflect lower baseline proptosis and variation in the clinical phenotype of TED. Some adverse events continued after treatment completion, highlighting the importance of long-term safety monitoring.
