Purpose

To present a rare and previously undocumented case of a bulbar subconjunctival dermoid cyst in a pediatric patient, emphasizing its significance in the differential diagnosis of conjunctival and orbital masses.

Methods

A 9-year-old child presented with a history of redness, irritation, burning, and pain in the right eye. Ophthalmological examination revealed an eroded, elevated lesion with an irregular surface on the right upper bulbar conjunctiva. Due to the clinical suspicion of posterior or orbital extension, an orbital magnetic resonance imaging was performed to evaluate potential intraorbital involvement. The patient subsequently underwent complete excisional biopsy of the mass, followed by ocular surface reconstruction using an amniotic membrane graft with fibrin glue.

Results

Histopathological evaluation confirmed the diagnosis of a dermoid cyst, demonstrating classic ectodermal components, including hair follicles within the lesion wall. The postoperative course was smooth, and no recurrence or complications were observed during the follow-up period.

Conclusion

Bulbar subconjunctival dermoid cysts are extremely rare entities in the pediatric population and can clinically mimic other conjunctival or anterior orbital masses. The identification of ectodermal structures, such as hair follicles, remains the gold standard diagnostic clue. When managing atypical conjunctival lesions, especially those with suspected deeper involvement, appropriate radiological imaging combined with histopathological confirmation is crucial for precise surgical planning and successful outcomes.
