Unusual reflex tearing during micturition and defecation : a case report .

Purpose
We aim to discuss the potential mechanisms underlying this rare presentation and management of such unusual phenomena.
Methods

Results
Reflex tearing during micturition and defecation is an unusual occurrence, with only a few documented cases in the literature. We present a case of a 46-year-old female who has experienced reflex tearing during both micturition and defecation since childhood. The patient sought advice from multiple ophthalmologists but found no effective solution. Unlike previously reported cases, where alternative management approaches were described, we initiated treatment with botulinum toxin injections into the lacrimal glands. The patient reported significant symptomatic improvement following treatment.
Conclusion
Tearing during micturition is a rare but benign phenomenon. The connection between these two occurrences is mediated by the synchronous activation of an abnormal parasympathetic pathway. This unusual occurrence can cause social embarrassment for many patients. Clinicians should be aware of this phenomenon in order to treat patients effectively.
