Scarless Ptosis correction with simultaneous lid crease formation:

Purpose

With the advent of social media and selfie culture there is a high demand for minimally invasive procedures for ptosis correction and improvement in cosmesis. The presentation describes technique and outcomes of the minimally invasive conjunctival sparing posterior white-line approach for simultaneous ptosis correction, lid crease formation and correction of hollow sulcus.


Methods

A retrospective study was done on patients with Ptosis, with moderate to good levator function who underwent posterior white line approach ptosis correction procedure for past 9 years. All surgeries were done by a single surgeon using the same technique in a tertiary Oculoplastic referral center


Results

Total number of eyes were n=311 eyes of 200 patients who underwent conjunctival sparing posterior white line ptosis correction. Mean follow-up was 78.7 days. Mean MRD was 1.5mm and Mean Levator function was 12mm. There were 3 eyes with under-corrections and 1 eye with over-correction which were successfully retreated with the same approach. Many eyes were combined with other procedures such as upper eyelid blepharoplasty n=92 eyes, upper and lower blepharoplasty n=12 eyes, browlift in n=4 eyes and lower eyelid blepharoplasty n=2 eyes. Major complications total included n=2, with ocular surface disorder in n=1 eye, scleritis in n=1 eye who was later discovered to have autoimmune disease. The scleritis resolved with a course of topical and systemic steroids


Conclusion

Conjunctival sparing Posterior white line approach Ptosis correction with knot exteriorization is a minimally invasive technique with high success rate of 98.8 percent for correction of ptosis. It also gives good lid crease which is an important aesthetic aspect for East Asians. As the levator is advanced, the post septal fat pads are simultaneously advanced, correcting hollow sulcus defects. It offers excellent cosmesis, high success rate, predictable outcome, avoids skin incision and scarring. It is a quick procedure with fast learning curve and low complication rate.
