Bilateral Lower Eyelid Involutional Ectropion Repair with Full-Thickness Skin Graft and Lateral Canthopexy: A Surgical Video

Purpose

To demonstrate the surgical technique for correction of lower eyelid ectropion using full-thickness skin grafting combined with lateral canthopexy.


Methods

A 76 year old male patient was referred to the oculoplastics consult at Beatriz Ângelo Hospital with a two year history of progressive epiphora and conjunctival hyperemia. Clinical examination revealed a mild bilateral lower eyelid ectropion, more pronounced in the left eye, with inferior scleral show and anterior lamellar deficiency. Surgical correction included release of anterior lamellar tension, followed by placement of a full-thickness skin graft harvested from the inner aspect of the upper arm, and lateral canthopexy to restore horizontal and vertical eyelid support.
Key surgical steps, along with technical tips and critical considerations are highlighted in the video, that was recorded using a Samsung Galaxy S23 camera and edited using the application Filmora®.


Results

The procedure was completed successfully, functional and aesthetic outcomes were satisfactory, without intraoperative or early postoperative complications.


Conclusion

Combined full-thickness skin grafting and lateral strip canthopexy is an effective and reproducible technique for the management of cicatricial ectropion, addressing both vertical skin deficiency and horizontal laxity. This video provides a practical step-by-step guide.
