Entropion in the early postoperative period following transconjunctival lower blepharoplasty: A case series

Purpose
To describe immediate and early postoperative entropion following transconjunctival lower eyelid blepharoplasty with fat repositioning, with emphasis on early conjunctival changes as a potential warning sign, and to report its incidence and management.
Methods
Retrospective case series of six patients who developed entropion within seven days following transconjunctival lower eyelid blepharoplasty with fat repositioning at a secondary-level ophthalmology clinic between 2009 and 2026. All procedures were performed using a standardized retroseptal transconjunctival technique with orbital fat transposition into a subperiosteal pocket. Postoperative photographs were obtained at 24 hours and patients were followed at scheduled intervals.
Results
Six cases of early postoperative entropion were identified among 833 transconjunctival lower blepharoplasties performed (0.72% incidence). Mean patient age was 53.7 years (range 42-65 years), and 83.3% were female. All patients presented with a negative orbital vector and preexisting lower eyelid laxity. Entropion developed between postoperative days 1 and 7. A consistent conjunctival abnormality was observed in all cases, characterized by overlapping of the caudal conjunctiva over the cranial conjunctiva. Entropion was complete in 66.7% and partial in 33.3% of cases. Early cases responded to conservative management with conjunctival repositioning with or without temporary Frost suture placement. One excluded delayed case presenting at 6 weeks required surgical intervention including lysis of conjunctival adhesions and sectioning of fibrotic bands. At 6-month follow-up, all patients achieved complete resolution with satisfactory functional and cosmetic outcomes.
Conclusion
Early postoperative entropion is a rare but clinically significant complication following transconjunctival lower eyelid blepharoplasty with fat repositioning. Overlapping of the caudal conjunctiva over the cranial conjunctiva may represent an early warning sign that allows prompt conservative treatment and may prevent progression to cicatricial entropion.
