Removal of a Foreign Body Located at the Medial Wall of the Right Orbit in a Pediatric Patient

Purpose
To present the management of a retained intraorbital foreign body located in the medial orbital wall in a pediatric patient with persistent periorbital swelling and discharge following orbital trauma, and its removal via a transcaruncular orbitotomy approach.
Methods
A 1-year-9-month-old girl presented with a history of falling from a table together with a plate approximately 2 months earlier. At an outside center, the periocular skin laceration had been repaired with primary suturation, and the patient was subsequently followed conservatively. Due to persistent periorbital edema and discharge during the postoperative period, she received repeated courses of topical and systemic antibiotic therapy. After partial resolution of the edema, a subconjunctival foreign body was detected adjacent to the laceration site during examination. While superficial foreign body extraction was being performed, an intraoperative plain radiograph raised suspicion of an additional orbital foreign body, and the patient was referred to our clinic. Orbital computed tomography demonstrated a foreign body consistent with a plate fragment located at the medial wall of the right orbit. Surgical removal via transcaruncular orbitotomy was planned.
Results
The foreign body was successfully removed through a transcaruncular approach without intraoperative or postoperative complications. Postoperative follow-up demonstrated resolution of the periorbital edema and discharge.
Conclusion
In pediatric orbital trauma, the possibility of a retained intraorbital foreign body should be considered in cases with persistent inflammation, discharge, or recurrent infection. Glass and ceramic fragments may be overlooked during the initial evaluation. Orbital computed tomography plays a crucial role in diagnosis. Transcaruncular orbitotomy represents a minimally invasive and effective surgical approach for the removal of foreign bodies located at the medial orbital wall.
