Impact of Initial Clinical and Imaging Findings on Recovery in Dysthyroid Optic Neuropathy

Purpose

This study aims to identify factors influencing visual field recovery in DON patients, based on objective assessments of visual field improvement.


Methods

A retrospective review of medical records from patients diagnosed with thyroid eye disease at King Khaled Eye Specialist Hospital (KKESH) between June 2014 and January 2024 was conducted. Inclusion criteria were confirmed thyroid eye disease diagnosis and documented visual field assessments at presentation and during follow-up (minimum of 6 months). Exclusions were incomplete records, other ocular comorbidities affecting the visual field, or incomplete follow-up. Data collection included patient demographics, clinical assessments (visual acuity, intraocular pressure, proptosis, strabismus, lid retraction, diplopia, optic nerve head swelling, color vision, and afferent pupillary defect), and visual field testing using the Humphrey Field Analyzer. Statistical analysis was performed using R, with univariate and multivariate analyses identifying significant factors influencing visual field recovery


Results

The study included 82 eyes from 44 patients with thyroid eye disease, average age 47.83 years. Key clinical characteristics included proptosis (72.0%).Visual field indices at presentation showed a mean deviation (MD) of -7.38 dB and a visual field index (VFI) of 85.54%. Radiological assessments indicated significant extraocular muscle involvement and optic nerve compression in 12.2% of cases. Univariate analysis revealed significant associations between visual field recovery and baseline visual acuity (logMAR), optic neuropathy, and visual field stage. Multivariate analysis confirmed optic neuropathy as a significant predictor of reduced visual field recovery (OR: 0.310, p = 0.046).


Conclusion

The likelihood of visual field recovery is lowered when optic neuropathy is present on imaging, according to both univariate and multivariate studies. Higher LogMAR values (low visual acuity) and more advanced visual field stages at presentation are associated with inferior recovery probability on univariate analysis, and they are almost statistically significant on multivariate analysis.
