Midface Thread Implantation as Preparation for Lower Blepharoplasty

Purpose
To evaluate the efficacy of midface thread implantation for correction of malar region descent and tear trough deformity as a preparatory step prior to lower blepharoplasty
Methods
Six patients presenting with midface ptosis and lower eyelid fat pad herniation underwent percutaneous thread implantation for soft tissue support and structural reinforcement of the midface. Transconjunctival or transcutaneous lower blepharoplasty was performed 4–6 months later. Outcomes were assessed through clinical examination and standardized patient questionnaires.
Results
The technique resulted in sustained improvement in malar projection and softening of the tear trough deformity. Preoperative structural support reduced the extent of orbital fat excision required during subsequent blepharoplasty by approximately 30–40% and contributed to a smoother lid–cheek junction. The complication rate was low, and no significant adverse events were observed.
Conclusion
Midface thread implantation may represent a safe and effective preparatory procedure for lower blepharoplasty in selected patients. By reinforcing the midfacial framework before surgery, this approach may reduce the risk of postoperative contour irregularities and contribute to a more harmonious and durable aesthetic outcome.
