Staged correction of bilateral post-burn cicatricial lower eyelid ectropion in a child

Purpose

To present a clinical case of staged surgical correction of bilateral post-burn cicatricial lower eyelid ectropion in a 12-year-old patient with bilateral lagophthalmos and limited donor sites for skin grafting.


Methods

A 12-year-old boy presented with post-burn cicatricial changes of the face and body, bilateral lower eyelid cicatricial ectropion, and bilateral lagophthalmos. A staged reconstructive approach was selected. The first stage included correction of the right lower eyelid ectropion using a full-thickness skin graft harvested from the supraclavicular region, with temporary tarsorrhaphy for ocular surface protection and graft stabilization. Donor site selection was limited by extensive burn scarring. Three months later, the second stage was performed using a full-thickness postauricular skin graft for correction of the left lower eyelid ectropion.


Results

Both grafts healed well without ischemia, necrosis, or infection. Skin sutures were removed on postoperative day 10, and tarsorrhaphy sutures after 3 weeks. Successful correction of bilateral cicatricial ectropion was achieved, with improved eyelid closure and ocular surface protection.


Conclusion

Staged reconstruction with full-thickness skin grafts is an effective approach for bilateral post-burn cicatricial ectropion in children. Careful donor site selection, temporary tarsorrhaphy, and close postoperative follow-up contributed to favorable anatomical and functional outcomes.
