Unilateral Lower Eyelid Retraction on upgaze: Case report of a rare form of synkinesis with review of literature.

Purpose
to highlight the manifestations of congenital lower eyelid retraction on upgaze as a form of cranial dysinnervation disorder
Methods
In this paper, we highlight the findings of 3 cases with congenital eyelid retraction on upgaze
Results
Out of 3 cases, 2 were observed since non-significant on primary gaze, one had significant eyelid retraction on primary position, which was more apparent on supraduction, lower eyelid spacer graft ( scleral) with retractor disinsertion, and canthal suspension was performed with satisfactory results
Conclusion
Only three other cases were previously reported in the literature for active, paradoxical lower eyelid retraction during upgaze. The mechanism behind eyelid retraction in previously published cases and our patients remains not fully identified. The authors hypothesize that these rare instances might indicate an undetected form of congenital oculomotor nerve synkinesis. Given the complexity and rarity of reporting this condition, MRI is essential in such cases to identify any anomalous orbital structures and guide appropriate surgical intervention.
