EXTERNAL DACRYOCYSTORHINOSTOMY AS A METHOD OF TREATING CHRONIC DACRYOCYSTITIS

Purpose
Chronic dacryocystitis is a condition of the lacrimal sac that compresses the tear drainage system, thereby leading to recurrent infections of the region. A common complication is acute dacryocystitis. External dacryocystorhinostomy (DCR) is one of the principal surgical methods, widely practiced across the world.
Methods
We present outcomes in four patients diagnosed with acute dacryocystitis. All had a long-standing history of the disease with frequent relapses of the acute phase. Clinical signs in all cases included swelling in the area of the lacrimal sac extending toward the lower eyelid, redness of the affected region, pain, purulent discharge, and severe discomfort. Patients underwent a three-week regimen of systemic and topical antibiotic and corticosteroid therapy. After a one-week symptom-free interval indicating resolution of active inflammation, patients were prepared for surgery under general anesthesia. External dacryocystorhinostomy with bicanalicular silicone tube implantation was performed. Postoperatively, patients were regularly monitored for 10 weeks, after which the bicanalicular silicone tubes were removed.
Results
The results demonstrated complete resolution of chronic dacryocystitis, reestablishment of tear drainage without recurrence or residual complications. The surgical wound area appeared unremarkable from the first postoperative follow-up. Skin sutures were removed on the 10th postoperative day, with proper wound edge healing. At the 10-week follow-up, after removal of the bicanalicular silicone tubes, patency of the newly established tear duct was confirmed by canal irrigation and a positive Jones dye test. Patients have been followed for an additional 12 months without any noticeable complications.
Conclusion
Based on the presented outcomes, external dacryocystorhinostomy with bicanalicular silicone tube implantation is a reliable method for treating chronic dacryocystitis. Results from the 12-month follow-up period indicate sustained patency of the lacrimal pathways without signs of inflammatory reactivation.
