Epithelioid Hemangiomas of the Orbit: A Case Series

Purpose

This study aims to describe the clinical features, management, and long-term course of orbital EHs, including the first reported case treated with rituximab.


Methods

A review of 272 orbital lesions was performed at the MUHC–McGill University Ocular Pathology Laboratory from January 2006 to August 2025. Among these 272 orbital lesions, three were histopathologically confirmed as EH. Data collected included demographics, presenting features, exophthalmometry, lesion location and imaging characteristics, management, and clinical course.


Results

Three cases (1.10% of orbital lesions) were identified. The mean age at presentation was 41 years (range, 23–67), and two patients were female. All cases were unilateral, with right-sided involvement in two. Lesions were intraconal (n=1), extraconal (n=1), or located along the superolateral orbital rim (n=1). Presenting symptoms included proptosis (n=2) and a painless mass (n=1). The mean interocular exophthalmometry difference was 7 ± 7 mm (range, 0–13 mm), and the mean largest basal diameter was 2.93 ± 0.55 cm. Two lesions involved adjacent structures, including complete optic nerve encasement and temporalis muscle infiltration. The intraconal lesion was partially excised and remained stable over 12 years. The extraconal lesion showed no response to systemic corticosteroids but decreased in size following rituximab therapy, the patient was lost to follow-up at 3 years. The superolateral lesion was excised with no recurrence at 1 year.


Conclusion

Orbital EH is rare and presents variably. Surgical excision remains the standard approach; however, in this series, one case showed a favorable response to rituximab, and another remained stable despite incomplete excision, suggesting potential alternatives when complete resection is not feasible. Further studies are needed to define optimal management.
