Evaluation of Systemic Inflammatory Markers in Patients with Congenital Nasolacrimal Duct Obstruction

Purpose

To evaluate systemic inflammatory marker levels in congenital nasolacrimal duct obstruction (CNLDO) cases and to compare with healthy controls


Methods

A retrospective review was conducted on patients underwent probing for CNLDO and healthy control subjects. Levels of neutrophils, monocytes, lymphocytes, and platelets were obtained from blood samples. Systemic inflammatory markers, including neutrophil-lymphocyte ratio (NLR), platelet-lymphocyte ratio (PLR), systemic immune-inflammation index (SII), systemic inflammatory response index (SIRI), systemic inflammation modulation index (SIMI), and aggregate systemic inflammation index (AISI) were calculated and compared between the two groups. In case of significant difference presence, the receiver operating characteristic curve (ROC) analysis performed to determine the best cutoff value of systemic inflammatory markers in CNLDO.


Results

In total, 85 CNLDO cases and 78 healthy controls were included in the study. CNLDO patients had significantly higher medians of SII (284.75 vs 213.54, p=0.014), SIRI (0.46 vs 0.36, p=0.021), SIMI (55.68 vs 50.81, p=0.047), and AISI (165.55 vs 107.94, p=0.010). The area under the curve of SII, SIRI, SIMI, and AISI in differentiating patients with CNLDO and controls was 0.564, 0.553, 0.534 and 0.576, respectively, according to ROC analysis. The best cutoff values (with sensitivity and specificity) were 123.32 (87.1% and 28.2%) for SII, 0. 182 (91.8% and 21.8%) for SIRI, 32.60 (87.1% and 32.1%) for SIMI, and 65.93 (90.6% and 30.8%) for AISI.


Conclusion

Patients with CNLDO exhibit slightly higher levels of blood-count-derived inflammatory markers, suggesting a possible link between systemic subclinical inflammation and CNLDO development. Further research with larger cohorts and broader inflammatory marker analysis is needed to elucidate the role of systemic inflammation in CNLDO pathogenesis.
