Purpose

To clarify benefits and indications of endonasal approach in the decompression of the orbital apex.

Methods

Report of a case-series of patients with compression of the orbital apex who underwent endonasal orbital decompression. The underlying pathologies of the compression are dysthyroid, lesions of the apex (cavernous haemangioma, mucocele, …) or hematoma.

Results

Good results are achieved with endonasal apical decompression, alone or in combination with external approach. Beside local expertise, decision is based on the presentation of the disease, the severity of the compression, the anatomy of the sinuses, the comorbidities in the sinuses, and the ocular status.

Conclusion

The benefits of endonasal approach of the apex are direct visualisation, very posterior access, approach of the sinuses and minimal peroperative pressure on the globe. Suggested indications of endonasal decompression of the orbital apex are insufficient anterior access to the apex, associated pathologies of the sinuses, need in maximum apical decompression, and interest in minimizing direct orbital surgical trauma.
