Lagophthalmos and Ocular Surface Exposure in Möbius Syndrome: Clinical and Surgical Considerations

Purpose
To present the ophthalmic manifestations and management challenges in two adult patients with Möbius syndrome, with emphasis on lagophthalmos, exposure keratopathy, and indications for surgical treatment.
Methods
We present two female patients with clinical features consistent with Möbius syndrome. Ophthalmic evaluation included assessment of eyelid closure, blink function, ocular surface status, and degree of lagophthalmos. One patient was examined in our clinic due to symptoms related to ocular surface exposure. The second patient underwent surgical management at an external center by a senior oculoplastic surgeon, with available clinical and intraoperative photographic documentation.
Results
Both patients demonstrated facial nerve palsy with incomplete eyelid closure and impaired blink reflex, resulting in chronic ocular surface exposure. Clinical findings included varying degrees of lagophthalmos and signs consistent with exposure keratopathy. In one patient, the severity of exposure findings supported the indication for surgical intervention aimed at improving corneal protection. In the second patient, surgical treatment was performed to improve eyelid closure and reduce the risk of corneal complications.
Conclusion
Möbius syndrome is associated with significant functional and reconstructive challenges due to facial nerve dysfunction and impaired eyelid dynamics. Careful assessment of ocular surface exposure is essential in determining the need and timing for surgical intervention. Early recognition and appropriate management are important for prevention of sight-threatening corneal complications.
