Periocular Dirofilariasis Presenting as a Migratory Eyelid Mass

Purpose
To present a rare case of periocular subcutaneous Dirofilaria repens infection, highlighting the diagnostic challenges and surgical management of a parasitic lesion in the periocular region.
Methods
We report the case of a 37-year-old woman presenting with migratory periocular swelling and discomfort in the right eyebrow region. Clinical examination revealed a linear, firm, subcutaneous lesion measuring 15 × 2 mm. Laboratory investigations demonstrated eosinophilia. Surgical treatment was performed through a linear skin incision with extraction of a parasite approximately 10 cm in length, followed by excision of adjacent necrotic tissue and skin adaptation. The extracted parasite was submitted for morphological identification.
Results
Parasitological examination confirmed the diagnosis of dirofilariasis. Following consultation with a parasitologist, a short course of albendazole therapy was initiated. The postoperative course was uneventful, with good functional and cosmetic outcome.
Conclusion
Periocular dirofilariasis is a rare but clinically significant condition that may mimic inflammatory or tumor-like lesions of the eyelids and orbit. Early recognition and surgical management are essential for definitive diagnosis and prevention of complications. Increased awareness among ophthalmologists is important for timely diagnosis and appropriate treatment of such cases.
