Adipose-Derived Stem Cell Therapy for Severe Refractory Ocular Surface Disease: A Four-Case Series

Purpose
To report the use of adipose-derived stem cell therapy as an adjuvant regenerative approach in four patients with severe refractory ocular surface disease.
Methods
Four patients with complex ocular surface disease unresponsive to conventional treatment underwent subconjunctival adipose-derived stem cell infiltration. In selected cases, treatment was combined with amniotic membrane transplantation or eyelid surgery. Outcomes were assessed according to conjunctival healing, corneal re-epithelialisation, ocular surface stability, recurrence of corneal erosions and functional improvement.
Results
Case 1 was a 63-year-old man with chronic ocular graft-versus-host disease, recurrent corneal erosions and neurotrophic ulceration. After previous corneal grafts and amniotic membrane transplantation, he developed severe conjunctival necrosis and was referred for possible evisceration. Following stem cell infiltration and amniotic membrane coverage, complete conjunctival closure was achieved in 5 days and corneal re-epithelialisation in 26 days, avoiding evisceration.Case 2 was a 68-year-old woman with advanced glaucoma, ectropion, severe ptosis and corneal damage after allergic reaction to dorzolamide. Despite ectropion repair and topical insulin, epithelial defects persisted. After stem cell injection and ptosis correction, ocular surface restoration and functional visual field improvement were achieved.Cases 3 and 4 were aged 72 and 85 years, both with limbal stem cell deficiency, recurrent corneal erosions and ocular surface instability. After treatment, both showed improved corneal surface regularity, better lubrication and fewer recurrent erosions.
Conclusion
Adipose-derived stem cell therapy may be a promising adjuvant treatment for severe refractory ocular surface disease. In this four-case series, it promoted surface regeneration, reduced recurrent erosions and improved anatomical and functional outcomes. Further studies are required.
