Purpose

Tumors of the lacrimal sac are exceedingly rare, particularly in the pediatric population, and provide diagnostic challenges that result in delayed detection. Herein, we report a rare case of Burkitt’s lymphoma presenting as acute dacryocystitis in a 15-year-old boy.

Methods

An extensive workup was carried out, including incisional biopsy, immunohistochemistry, Computed Tomography (CT) scan of the brain and orbit, Magnetic resonance imaging (MRI) of the brain and orbit, and whole-body positron emission tomography (PET).

Results

Tumor cells were positive for CD45, PAX5, CD10, BCL6, Bcl2, and Ki-67 by immunohistochemical analysis. The patient had three cycles of chemotherapy using the COG ANHL01P1 regimen after a thorough oncologic workup, which revealed no systemic metastases.

Conclusion

Lacrimal sac tumors, though rare, can initially masquerade as an inflammatory process such as dacryocystitis. Careful consideration of ocular signs and close follow-up, especially in children, is critical in such misleading cases.
