THE IDENTITY LIFT: A COMBINED UPPER BLEPHAROPLASTY AND MODIFIED BRASSIERE TECHNIQUE FOR NATURAL PERIOCULAR REJUVENATION WITH VOLUMETRIC PRESERVATION

Purpose
Upper eyelid blepharoplasty, when performed in isolation, risks undermining brow position and accelerating volume depletion — particularly in younger patients where facial identity preservation is paramount. This paper describes the “Identity Lift,” a structured surgical approach combining upper eyelid rejuvenation with brow support and targeted periocular volumisation, designed to restore a natural, rested appearance while preserving the patient’s intrinsic facial identity.
Methods
A retrospective review of consecutive patients undergoing the Identity Lift technique was performed. The procedure integrates three components: (1) conservative upper eyelid skin excision calibrated to brow position, (2) a reproducible modified brassiere suture utilising internal fixation to enhance sub brow lateral projection and re-establish the lid-brow relationship, and (3) selective periocular volumisation to address temporal hollowing and superior sulcus depletion. Standardised preoperative and postoperative photography was obtained in all cases. Outcomes were assessed using validated patient satisfaction scores and blinded clinician-rated photographic grading.
Results
300 patients, follow-up over 1 year, satisfaction scores 8/10, complication 10% between recurrence and asymmetry.
Preliminary results demonstrate consistent improvement in brow contour, upper eyelid crease definition, and periocular volume without the operated or hollowed appearance commonly associated with conventional blepharoplasty. Patient-reported satisfaction was high, with particular emphasis on preservation of natural appearance.
Conclusion
The Identity Lift offers a reproducible, anatomy-respecting framework for upper periocular rejuvenation that addresses the limitations of skin-excision-only blepharoplasty. By integrating brow support and volumisation as first-line considerations rather than adjuncts, the technique prioritises identity preservation — a clinically and aesthetically meaningful distinction in contemporary oculoplastic practice.
