Purpose
To describe the case of a 65 year old man who suffered severe facial trauma needing multiple facial surgeries using osteosynthesis plates and presenting to our ophthalmological clinic with a complete ptosis of his left upper eyelid and the left eye in exo- and hypotropia with a vertical gaze palsy.
Methods
We describe this case using facial photographs and radiological and surgical images, to illustrate the complexity of diagnosing this acurately and therefore being able to treat it efficiently.
Results
Postoperatively our patient had his ptosis corrected suficiently, demonstrating an unobstructed visual axis and showed orthophoria in straight gaze without diplopia with only a minimal impairment to abduct with the left eye.
Conclusion
We demonstrate the difficulty in diagnosing and treating a parcial 3rd nerve palsy with complete ptosis, showing that collaboration between the ophthalmic plastic and neuro-strabism surgeon is pivotal in the succes of these cases.
