EpiGARD as a Temporary Artificial Skin Substitute in Eyelid Reconstruction – Reviews and case reports

Purpose
To summarize the potential role of EpiGARD as a temporary artificial skin substitute in periocular reconstruction following oncologic eyelid surgery and to present our two clinical cases.
Methods
A focused literature review was performed using PubMed databases with the keywords: “EpiGARD,” “synthetic skin substitute,” “eyelid reconstruction,” “periocular reconstruction,” “Mohs surgery,” and “tumor excision.” Relevant studies, technical reports, and case series discussing temporary synthetic skin substitutes in periocular or oncologic reconstruction were reviewed. Our two cases include a female patient who received epiGARD coverage because complete R0 tumor-free resection could not be guaranteed, and a male patient due to significant intraoperative bleeding.
Results
Published evidence suggests that EpiGARD may provide effective temporary wound coverage following eyelid tumor excision. Its bilayer polyurethane structure offers mechanical protection, exudate control, and maintenance of a moist wound environment while minimizing adhesion to the wound surface. These characteristics facilitate atraumatic dressing changes and support granulation tissue formation prior to definitive reconstruction. Current evidence in oculoplastic surgery remains limited mainly to technical reports and small case series, with no large prospective comparative studies available.
Conclusion
EpiGARD represents a useful temporary synthetic skin substitute in selected cases of periocular reconstruction after tumor excision, particularly when staged reconstruction or delayed definitive closure is required. Although it lacks true dermal regenerative properties, its ease of use, atraumatic handling, and wound-protective characteristics make it a valuable adjunct in complex eyelid reconstruction. Further prospective studies are needed to better define its role compared with modern dermal regeneration templates and acellular dermal matrices.
